Coding for Coflex

New CPT codes were established in 2017 that represent the insertion of devices such as Coflex for interlaminar/interspinous process stabilization/distraction.

CPT code 22867 represents insertion of a Coflex device without fusion but with an open decompression and code 22868 is reported for the second level.

CPT code 22869 represents insertion of a Coflex device without fusion or open decompression and code 22870 is reported for the second level.

There are two important factors that need to be noted with these codes. First is the fact the Coflex is a non-fusion procedure, however there will be cases where the physician inserts a Coflex device as part of a spinal arthrodesis and in those instances you would report the Unlisted spine code 22899 and not a code from the 22867-22870 range or posterior instrumentation codes.

Note also the add-on codes 22868/22870 which state “second level”. Whenever a Coflex device is placed at three or more levels you should again report the Unlisted spine code 22899 for the entire procedure and not the primary and add-on codes 22867/22868 or 22869/22870.
